Supported Family Member or Technology Grant Application
The Supported Family Member Grant is to help provide financial support to offset the cost of items or services that can benefit the quality of life and development of an individual with Down syndrome. Items or services should be specific for individuals with Down syndrome. Eligible items or services may include educational products or classes, registration or membership fees, therapies, therapeutic, adaptive or safety equipment, conferences, learning opportunities or experiences (for parents and individuals). Ineligible items or services may include every day essential items, necessities, or consumer goods typically purchased for a person with or without a disability.
The Technology grant is to help assist the individual with Down syndrome by providing eligible devices or applications used for assistive or adaptive communication. It is understood that there are a wide variety of technology and software apps available for those with disabilities. Eligible items include iPads, laptops, and/or communication apps, and/or updates to maintain those apps.
An Assistive technology device is identified in the IDEA Sec. 300.5 as:
Restrictions: Each DSAW-Fox Cities supported family can apply for unlimited grants per calendar year up to the maximum of $500 award for the individual with Down syndrome. The technology grant can only be awarded once every 3 years if providing a device for the individual. The DSAW-Fox Cities’ grant application is not a guarantee of reimbursement. If you are unsure if a product or service will be eligible, please submit a request prior to purchase and your request will be reviewed. **Alternate funding resources should also be used before applying for the DSAW-Fox Cities grant award. Other funding sources include: insurance, school, IRIS, CLTS. Please provide other resources used and reasons for denial.**
Preference: There will be times when the DSAW-Fox Cities grants cannot meet all submitted requests. In these situations, preference will be given to requests that meet the following criteria:
Notification: DSAW-Fox Cities’ grants are reviewed by the President and the advisory board. We aim to notify you of the decision within 30 days of your request. You will receive a response of approval or denial via email (or US mail, if no email address is given). If you have not heard a response after that time, please contact us.
Payment: All grant requests must reimburse actual expenses. You will pay for the item or service first, send us a copy of your itemized receipt/s, and we will send you a reimbursement check. Please also include, where applicable, the enrollment confirmation for a class or conference.
**Deadline to submit receipts should be completed within a 60 day period after purchase, but not longer than the calendar year the item or service was purchased.**
* * * * A “DSAW-Fox Cities supported family” is defined as the immediate family/guardian of an individual with Down syndrome that is currently listed on the DSAW-Fox Cities database and residing in Outagamie, Winnebago, or Calumet County. If a family resides outside of these three counties and is not already a supported family of another DSAW chapter, this family must be actively participating, attending social events, or fundraising for DSAW-Fox Cities.* * * *

The term does not include a medical device that is surgically implanted, or the replacement of such a device.
